The Adobe Acrobat Forms posted on the website are designed to allow the user to FILL-IN and SAVE the form.  For your convenience a "print form" and "e-mail form" buttons have been placed on the form.  The "e-mail form" button will only function properly if your e-mail client (Outlook, Outlook Express, etc.)  is not  configured on your computer.  STOP.  So if your unsure or don't know.  SAVE the form after you fill it out.  Then either e-mail or print & fax the completed form to sales@wilsontrophy.com or 916-927-9955
